Health insurance:

While I am not an in-network provider with insurance panels, you may be able to submit claims for out-of-network coverage, depending on your benefits. I can provide you with a superbill which contains all the information your insurance will need.

Cancellation Policy:

I require 24 hours notice for cancellation or rescheduling of an appointment. No shows, cancellations, or reschedules within 24 hours of our appointment time will be billed to the client at the full session rate.

Good Faith Estimate Notice:

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
